I know it depends a bit on the characters and the races, but ageing
is actually also quite a nasty thing. Say you game-time played 10
years, so your PC would be in his 30ies if you started at TSR ages,
and then some nasty sth comes in and ages you to 40 years old. Qould
you be recognised? stats change only for the worse (Wis changes only
with 'real' time passed, not drained), options for adventuring cease,
do you have an heir? all these questions can come into the piscture,
pretty nasty I would say.

There is a world just around the corner of your mind
where reality is an intruder and dreams come true.
You may escape into it at will, you need no secret password,
magic wand or Alladins Lamp, all you need is your own imagination...

This would also be a good time to hit the PC with some of the other natural
afflictions of old age. Arthritis, cataracs, and other maladies that come
with old age can be used to make the experience more "real". Doing this is
sure to drive them in search of a way to reverse the aging, and with that
hook you can create loads of adventure. Can anyone say Fountain of Youth.
